THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our Commitment to Your Privacy Webwell Group, PLLC (WWG) is dedicated to maintaining the privacy of your Protected Health Information (PHI). In conducting our business, we will create records regarding you and the treatment and services we provide to you. We are required by law to maintain the confidentiality of health information that identifies you. We also are required by law to provide you with this notice of our legal duties and the privacy practices that we maintain in our practice concerning your PHI.

How We May Use and Disclose Your PHI We may use and disclose your PHI for treatment (coordinating care with other providers), payment (processing insurance claims), and health care operations (internal quality reviews). In 2026, federal regulations (42 CFR Part 2) were updated to further align the privacy of substance use disorder records with general HIPAA standards. This means that if SUD records are involved in your care, we will obtain your single, written consent for all future uses and disclosures for treatment, payment, and healthcare operations.

Your Rights Regarding Your PHI

  • Right to Inspect and Copy: You have the right to inspect and obtain a copy of your PHI. Under current 2026 “Right to Access” standards, we will provide these records in your preferred electronic format whenever possible and within 30 days of your request.

  • Right to Amend: If you feel the PHI we have is incorrect, you may ask us to amend the information.

  • Right to an Accounting of Disclosures: You may request a list of certain non-routine disclosures we have made of your PHI.

  • Right to Request Restrictions: You may request a restriction on the PHI we use or disclose for treatment, payment, or operations.

Complaints If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer at [email protected] or with the Secretary of the Department of Health and Human Services. You will not be penalized for filing a complaint.
